Client Consent Forms

Before we get started, we need two completed forms from each applicant. They're required for every file.

Service Agreement

Required for each applicant

The Service Agreement sets out the terms of the mortgage brokerage services provided through Mortgage Connection Inc. and important information about the mortgage process. Each applicant completes and signs their own agreement.

What it covers

  • Our role as your mortgage broker
  • How we're paid
  • Your rights and responsibilities as a borrower
  • Privacy and consent to collect and use your information
Sign Service Agreement

If you're applying jointly, both of you need to complete this.

PEP Declaration

Required for each applicant

Complete this after the Service Agreement. It's a separate form.

Canadian financial regulations require us to confirm whether any applicant, or a close family member or associate, is a Politically Exposed Person (PEP). It doesn't affect your ability to qualify.

What is a PEP?

An individual who holds or has held a prominent public position, such as a head of state, senior government official, military officer or senior executive of a state owned enterprise. It extends to certain family members and close associates.

Select the option that applies to you:

I am NOT a Politically Exposed Person I AM a Politically Exposed Person

Unsure? Select "I AM" and complete the extra disclosure. We'll follow up if anything else is needed.

Once both forms are signed, you're all set. Send your documents to docs@lacroixmortgage.com or upload them to your secure Dropbox folder. Your broker will send you the link. Not sure which documents you need? Build your document checklist.
